VUR is a condition where urine flows backward from the bladder toward the kidney instead of only downward and out, due to a weakness in the valve where the ureter meets the bladder. It raises the risk of kidney infection.

Does VUR always need surgery?

No. Most children, especially with lower grades of reflux, are managed with monitoring and prompt infection treatment, and the reflux often resolves on its own as the child grows. Surgery is considered for higher grades or recurrent infections.

How is VUR diagnosed?

It's diagnosed with a specialised bladder X-ray study, usually done after an initial urinary tract infection or an ultrasound finding, such as antenatal kidney swelling.

Will my child outgrow VUR?

Many children, particularly with milder grades, outgrow reflux over time as the valve mechanism matures. Regular follow-up confirms whether it is improving.

Vesicoureteral reflux — usually just called VUR — sounds alarming the first time a parent hears it, but for most children it is a manageable, often self-resolving condition, not a reason for panic.

What Is VUR?

Normally, urine flows in one direction: down from the kidneys, through the ureters, into the bladder, and out. In VUR, a weakness in the valve where the ureter meets the bladder allows urine to flow backward — up toward the kidney — particularly when the bladder is full or during urination. This backward flow can carry bacteria toward the kidney, raising the risk of kidney infection.

How It's Diagnosed

VUR is usually discovered after investigating a child's first significant urinary tract infection, or sometimes on antenatal scans showing kidney swelling. The specific test used to diagnose and grade reflux is a specialised bladder X-ray study, done after the initial ultrasound.

Grades of VUR

Reflux is graded from 1 (mild, urine reaches only the lower ureter) to 5 (severe, with significant swelling of the ureter and kidney). The grade — along with the child's age and how often infections occur — guides the management plan.

Management: Conservative vs Surgical

Lower grades (1–3): usually managed with monitoring, prompt treatment of any infections, and sometimes a low-dose preventive antibiotic; many children outgrow the reflux as they grow
Higher grades (4–5), or reflux that doesn't improve: may need a surgical or endoscopic procedure to correct the valve mechanism, particularly if infections keep recurring despite monitoring

The goal of managing VUR is never to treat the reflux itself for its own sake — it is to protect the kidneys from repeated infection while the reflux has a chance to resolve on its own, which it does in the majority of milder cases.

What Parents Can Do

Prompt treatment of any UTI, good hydration, regular toilet habits (not "holding" urine for long periods), and treating constipation if present (which can affect bladder function) all support the natural resolution of reflux and reduce infection risk while your pediatric urologist monitors progress.
