Testicular Torsion in Boys — The Groin Emergency Parents Must Never Ignore

What Testicular Torsion Is

Testicular torsion happens when the testis twists on the cord that supplies its blood, cutting off circulation. Without blood flow, testicular tissue begins to die within hours. It can occur at any age from infancy through the teenage years, but is most common around puberty.

Recognising the Symptoms

• Sudden, severe pain in one testicle or the groin — often starting abruptly, sometimes waking a boy from sleep
• Swelling, redness, or a testicle sitting higher or at an unusual angle
• Nausea or vomiting alongside the pain
• Pain that may also be felt in the lower abdomen

Torsion can sometimes be confused with other causes of groin pain, but the safe rule is simple: any sudden, severe testicular or groin pain in a boy needs emergency evaluation, every time, without waiting to see if it's "probably nothing."

Why Every Hour Counts

Once the blood supply is cut off, testicular tissue can be saved with prompt surgery in the first several hours. Beyond that window, the risk of losing the testis rises significantly. There is no home remedy, no medication, and no reason to wait — this can only be fixed with emergency surgery to untwist (and usually fix) the testis.

What Happens at the Hospital

A boy presenting with sudden testicular pain is assessed urgently. If torsion is suspected on examination, emergency surgery to explore and untwist the testis is usually performed without delay — waiting for extensive imaging can waste the limited window available. During surgery, the affected testis is fixed in place to prevent it twisting again, and, since torsion can occur on the other side later in life, the unaffected testis is usually fixed at the same time as a preventive measure.

After Treatment

If treated within the golden window, most testes are saved and go on to function normally. If treatment is delayed beyond several hours, the testis may not be salvageable and may need to be removed — which is why acting immediately, every time, matters so much more here than with almost any other pediatric surgical condition.
