What Is Tension Pneumothorax?

Tension pneumothorax is a life-threatening medical emergency. It happens when air enters the space between the lung and chest wall (pleural space) and cannot escape. The trapped air continues to build up with each breath, putting increasing pressure on the heart and great blood vessels. Without immediate treatment, it can be fatal within minutes.

Diagnosis

Tension pneumothorax is a clinical diagnosis β€” doctors treat immediately based on symptoms rather than waiting for X-ray. This is because delay can be fatal. In hospital, chest X-ray confirms the diagnosis after initial treatment.

Emergency Treatment

Needle decompression is the first emergency treatment β€” a large needle is inserted into the chest to release trapped air. This buys time for definitive treatment.

Chest drain (intercostal drain) is then inserted to fully drain the air and allow the lung to re-expand. The patient is continuously monitored in the ICU.

After Treatment

Most patients recover well with prompt treatment. The chest drain typically remains in place for 24–48 hours while the lung re-expands. Underlying causes such as trauma or lung disease are addressed. Follow-up chest X-rays confirm full lung expansion.
