"I have gallstones, but I feel fine β€” do I really need surgery?" This is one of the most common questions in general surgery, and the honest answer is: it depends.

What Are Gallbladder Stones?

Gallstones are hardened deposits that form in the gallbladder, a small organ beneath the liver that stores bile used to digest fat. Many people carry gallstones for years without ever knowing, discovered only incidentally on a scan done for another reason.

Symptoms That Suggest Gallstones Are Causing Problems

Sudden, severe abdominal pain with fever, or jaundice, needs urgent medical attention β€” these can indicate a blocked bile duct or gallbladder infection, not simple indigestion.

When Is Surgery Recommended?

Surgery is generally advised when gallstones are actually causing symptoms β€” repeated pain attacks, inflammation of the gallbladder, or complications like a blocked duct or infection. Silent gallstones causing no symptoms are sometimes simply monitored, particularly if found incidentally.

Surgery is more strongly recommended if you have diabetes, since gallbladder infections tend to be more serious and progress faster in diabetic patients.

What the Surgery Involves

The standard treatment is removal of the gallbladder (cholecystectomy), most often performed laparoscopically through a few small incisions. The gallbladder is not essential for life β€” bile simply flows more directly from the liver to the intestine afterwards, and most people notice no meaningful difference in digestion.

Recovery from laparoscopic gallbladder removal is usually quick, with most patients home within a day or two and back to normal activity within one to two weeks.

Why "Repeated Attacks" Change the Decision

Each attack of gallbladder pain carries a real risk of the next one being a genuine complication β€” an infected, inflamed gallbladder, or a stone lodging in the bile duct. Repeated attacks are usually the point where the balance shifts clearly in favour of surgery rather than continued monitoring.

Can Diet Alone Dissolve Gallstones?

Dietary changes can reduce how often symptoms flare up, but they do not reliably dissolve existing stones. Once stones are established and causing symptoms, surgery remains the definitive treatment.
