What Are Asthma Flareups?
Asthma flareups β also called asthma attacks or exacerbations β are episodes when asthma symptoms suddenly get worse. During a flareup, the airways become inflamed, swollen, and produce extra mucus. This makes breathing very difficult. Flareups can range from mild to life-threatening.
Common Triggers
- Allergens β dust mites, pet dander, pollen, cockroaches, mould
- Respiratory infections β viral colds, flu, COVID-19
- Air pollution β vehicle exhaust, industrial smoke, burning crop waste
- Exercise β exercise-induced bronchoconstriction
- Weather changes β cold air, humidity changes
- Strong smells β paint, perfume, cleaning products
- Emotions β stress, laughter, crying
- Medications β aspirin, NSAIDs, beta-blockers in some patients
- Gastro-oesophageal reflux β acid reflux can worsen asthma
Recognising Severity
Mild flareup: Mild wheeze, able to speak full sentences, responds to rescue inhaler within 20 minutes.
Moderate flareup: Noticeable wheeze, only able to speak partial sentences, rescue inhaler provides partial relief.
Severe flareup: Severe difficulty breathing, cannot complete sentences, rescue inhaler not helping, respiratory rate above 30/minute.

Diagnosis
Asthma is diagnosed with: detailed medical history and symptom pattern, spirometry (lung function test), peak flow measurement, allergy tests, and chest X-ray to rule out other conditions.
Management of a Flareup at Home
- Sit upright β do not lie down
- Use your rescue (reliever) inhaler β 2β4 puffs every 20 minutes
- Stay calm β anxiety worsens breathlessness
- If no improvement after 3 doses β seek emergency care
- Keep a record of your symptoms and triggers
Long-Term Prevention
- Take preventer (controller) inhaler daily as prescribed
- Identify and avoid your personal triggers
- Have a written Asthma Action Plan from your doctor
- Monitor peak flow readings at home
- Annual flu vaccination
- Regular review with your doctor every 3β6 months
