Antenatally Detected Kidney Swelling (Hydronephrosis) — What to Do After the Scan

Hearing the word "hydronephrosis" at a routine pregnancy scan is unsettling for any parent — but in the majority of cases, this common finding resolves on its own and needs nothing more than a follow-up scan after birth.

What the Scan Finding Means

Hydronephrosis simply means the kidney's collecting system — where urine gathers before draining down the ureter — appears mildly to moderately swollen with fluid on ultrasound. It is one of the most commonly detected findings on routine antenatal scans, seen in a meaningful minority of pregnancies, and most cases are mild.

What Happens After Birth

Babies with antenatal hydronephrosis are typically followed up with a repeat ultrasound in the first few weeks of life (timing depends on how significant the antenatal finding was). This confirms whether the swelling has resolved, stayed the same, or needs closer attention.

• Mild cases: often resolve completely and need no further action beyond the confirmatory scan
• Moderate cases: may need periodic ultrasound monitoring over the following months
• More significant cases: may need further specialised imaging to understand the cause

When Further Tests Are Needed

A pediatric urology review decides whether additional imaging — such as a specialised bladder X-ray study or a kidney function scan — is needed, based on how significant the swelling is, whether it affects one or both kidneys, and how it changes over the follow-up scans.

When Surgery Is Considered

Only a minority of babies with antenatal hydronephrosis ultimately need any intervention. Surgery is considered when there is a true blockage at the point where the kidney drains into the ureter (a condition called pelvi-ureteric junction obstruction), confirmed by imaging and, if needed, kidney function testing — not simply because swelling was seen on a scan.

What Parents Can Do While Waiting

Keep all scheduled follow-up ultrasound appointments even if your baby seems completely well — hydronephrosis causes no symptoms in most infants, so the scan is the only way to track it. Watch for fever with no obvious cause, which can be a sign of urinary infection and should be checked promptly.
